Athletic Requirements

1. ATHLETIC PHYSICAL - Physical forms are required by LAUSD for every student who wishes to participate in interscholastic athletics.

 

A. The FRONT of physical form is to be carefully completed by the student and his/her parent(s) or legal guardian(s) before participation
in interscholastic athletics. The multiple questions should be answered YES or NO. All YES answers should be explained.

 

B. The BACK of physical form is to be completed by physician or staff after history and consent forms are completed.

 

C. Physicals are valid for 12 months after doctors visit.

 

2. ATHLETIC CLEARANCE - https://athleticclearance.com/

 

A. Complete the online application (one per season)

 

B. Create an account

 

C. Complete all required areas

 

D. Submit signed confirmation page received once online process is completed

 

3. 2.0+ GPA at the end of each quarter.
